The Formula Header user form contains below details.
Three fields, Organization, Formula and Formula Version, are available to the user and all of them are mandatory.
The read-only fields Formula Description and Status are for the additional information purpose. The values in these fields are populated when a formula version is selected in the Formula Version field.
